New patients should bring copies of the privacy (HIPAA) form, medicare waiver if medicare, patient demographic form, and general medical history form. Just click the appropriate link below and print the forms. Filling them out at home will reduce your wait in our office.
If you are seeing Dr. Ark or Gecha, use their specific forms and NOT the General Medical History Form.
